Patient Portal Registration

Please Note: Our Patient Portal Registration is password protected. If you do not have the password from the Intake Department, please return to our home page and click on the “New Patients” button to complete an Intake Inquiry form. If you are already involved in our Intake process and you have the Portal Registration password, please follow the directions below:

Please click on the “Portal Registration” button where you will be directed to enter the password provided to you by the Intake department. You will then be directed to our Patient Portal Registration where you must complete the assigned documents before we can schedule your first session.
